Mainstream theories in the clinical ethics literature markedly differ from the living bioethics in clinical practices. This discussion paper considers how seldom recognised theories influence clinical ethics committees. A companion paper examined four major theories in social science: positivism, interpretivism, critical theory and functionalism, which can encourage legalistic ethics theories or practical living bioethics, which aims for theory–practice congruence. This paper develops the legalistic or living bioethics themes by relating the four theories to clinical ethics committee members’ reported aims and practices and approaches towards efficiency, power, intimidation, justice, equality and children’s interests and rights. Different approaches (...) to framing ethical questions are also considered. Being aware of the four theories’ influence can help when seeking to understand and possibly change clinical ethics committee routines. The paper is not a research report but is informed by a recent study in two London paediatric cardiac units. Forty-five practitioners and related experts were interviewed, including eight members of ethics committees, about the work of informing, preparing and supporting families during the extended process of consent to children’s elective heart surgery. The mosaic of multidisciplinary teamwork is reported in a series of papers about each profession, including this one on bioethics and law and clinical ethics committees’ influence on clinical practice. The qualitative social research was funded by the British Heart Foundation, in order that more may be known about the perioperative views and needs of all concerned. Questions included how disputes can be avoided, how high ethical standards and respectful cooperation between staff and families can be encouraged, and how minors’ consent or refusal may be respected, with the support of clinical ethics committees. Traditional mechanistic accounts of language processing derive almost entirely from the study of monologue. Yet, the most natural and basic form of language use is dialogue. As a result, these accounts may only offer limited theories of the mechanisms that underlie language processing in general. We propose a mechanistic account of dialogue, the interactive alignment account, and use it to derive a number of predictions about basic language processes. The account assumes that, in dialogue, the linguistic representations employed by the (...) interlocutors become aligned at many levels, as a result of a largely automatic process. This process greatly simplifies production and comprehension in dialogue. After considering the evidence for the interactive alignment model, we concentrate on three aspects of processing that follow from it. It makes use of a simple interactive inference mechanism, enables the development of local dialogue routines that greatly simplify language processing, and explains the origins of self-monitoring in production. We consider the need for a grammatical framework that is designed to deal with language in dialogue rather than monologue, and discuss a range of implications of the account. Currently, production and comprehension are regarded as quite distinct in accounts of language processing. In rejecting this dichotomy, we instead assert that producing and understanding are interwoven, and that this interweaving is what enables people to predict themselves and each other. We start by noting that production and comprehension are forms of action and action perception. We then consider the evidence for interweaving in action, action perception, and joint action, and explain such evidence in terms of prediction. Specifically, we assume (...) that actors construct forward models of their actions before they execute those actions, and that perceivers of others' actions covertly imitate those actions, then construct forward models of those actions. We use these accounts of action, action perception, and joint action to develop accounts of production, comprehension, and interactive language. Importantly, they incorporate well-defined levels of linguistic representation. We show how speakers and comprehenders use covert imitation and forward modeling to make predictions at these levels of representation, how they interweave production and comprehension processes, and how they use these predictions to monitor the upcoming utterances. We show how these accounts explain a range of behavioral and neuroscientific data on language processing and discuss some of the implications of our proposal. (shrink)

Two studies employed a dissociative detachment induction technique to examine if experiences of dissociation increased acute shame feelings. Study 1 recruited college participants, while Study 2 enlisted adults attending treatment for childhood sexual abuse. Two hypotheses were explored: (1) more shame would be reported following a dissociative detachment induction than a relaxation induction; and (2) shame would increase when detachment was induced in the relationship context of a close other than when alone. Study 1 (N = 81) effectively induced detachment (...) and participants reported higher shame in this condition compared to the relaxation condition. This effect was maintained when state anxiety was controlled. The relationship context produced no impact on dissociation or shame. Attributions around feeling flawed predominantly linked detachment experiences with subsequent shame feelings. In Study 2 with clinical participants (N = 28), regression analyses showed state shame was predicted by acute detachment after controlling for state anxiety, gender, and trait shame and dissociation. The most common appraisals offered for why detachment led to feelings of shame was being flawed and exposed. Collectively, our findings suggest that increased acute shame results from detachment experiences, making more specific the relationship between shame and dissociation. (shrink)

The prospect of shared decision-making with animals is an elusive one. Its elusiveness comes largely from how difficult it is to assess the linguistic abilities of animals, whether that be their ability to ‘speak’ or their ability to maintain propositional values. In this paper, I suggest a path to shared decision-making with animals that attempts to avoid these deadlocks by using resources from biosemiotics and _Umwelt_ theory. I begin with an examination of the general structure of decision-making, demonstrating its future-orientation, (...) comparison of imagined futures, and assessment of what things matter to participants in decision-making. Animals’ capability of having things matter to them, due to their residence in _Umwelten_, offers a means to shared decision-making with animals via a process I call ‘imaginative adjuncting.’. The requirement of total evidence is a mainstay of Bayesian epistemology. Peter Fisher Epstein argues that the requirement generates mistaken conclusions about several examples that he devises. Here we examine the example of Epstein’s that we find most interesting and argue that Epstein’s analysis of it is flawed.

Lucas proposed that Godel's theorem made possible a refutation of mechanism—the thesis that mind is wholly comprehensible as a consistent, rule-governed machine. A sympathetic reading of Lucas's argument might run something as follows: ‘If I am a machine then it will be possible in principle to give a specification of the consistent formal system, L, that represents me. If this formal system were handed to me, I would be able to prove a (...) Gödel sentence, G, which L could not generate—that is, L could not model my proving G. The tension between a literary-critical approach to the text versus a dogmatic understanding is exposed, as well as the challenge of a political ideology of power. Geyser’s understanding and application are measured in terms of the current state of New Testament exegetical research. The recognition of extracellular molecules by cell surface receptors is the principal mechanism used by cells to sense their environment. Consequently, signals transduced as a result of these interactions make a major contribution to the regulation of cellular phenotype. Historically, particular emphasis has been placed on elucidating the intracellular consequences of growth factor and cytokine binding to cells. In addition to these interactions, however, cells are usually in intimate contact with a further source of complex structural and functional information, namely (...) immobilised extracellular matrix and/or cell surface adhesion proteins. A key question in recent years has been whether cells use the myriad of adhesion protein‐receptor interactions purely for structural and migratory function, or whether these interactions also make a more varied contribution to cell phenotype. Here we review dynamic aspects of the function of one major class of adhesion receptor, the integrins. In particular, we focus on the evidence for shape changes in integrin molecules, the mechanisms responsible for regulating ligand binding, and the signals transduced following integrin occupancy.
